Information you submit is stored for the Oklahoma Next Level team to review nominations, respond to inquiries, discuss partnerships, maintain the launch list, and process shirt order requests. Shirt order requests include your selections, contact details, and shipping ZIP code; payment is arranged separately. Contact details and submissions are not displayed publicly on this site.

Forms are for adults age 18 and older. Do not submit private medical details, home addresses, student records, or other sensitive information. Nominating an athlete is not permission to publish their story. Interview and publication permissions are handled separately, including parent or guardian authorization for minors.
